Comprehensive Eye Exam
- Complete the appropriate Patient Form – whether you are a New Patient to our practice or a Returning Patient.
- Review the information on the Optomap
- Bring your insurance card
- If you wear contact lenses, please bring your glasses with you
- If you are a new patient and have information about your current contact lenses, please bring that with you
Contact Lens Check Up
- If your contact lenses are not overnight lenses, please put them in as soon as you can in the morning, or at least 2 hours prior to your appointment, so they have time to settle before your appointment
