Pre-Appointment Checklist

1. Completed patient information forms
2. Insurance Card(s)
3. Medication List
4. Glasses (even if you wear contacts)
5. Any previous medical records including your current contact lens prescription.

Arrive early to fill out paperwork

Please plan to arrive 10 minutes ahead of your appointment if you need to fill out your paperwork at the office.
